Kindergarten, 2nd and 6th Grade Students
Please note that by May 1st, 2024 you must show proof that your child has seen the dentist within the past 18 months. This is a State of Illinois requirement. If the Smile Illinois dentist examined your child, when they were here on campus, you have fulfilled this obligation.
5th grade Students
Please note you can turn in a physical exam and immunization records for 6th grade requirement anytime in person or via email. As long as the physical exam took place after 8/15/2023.
6th Grade Students
If you want to participate in sports in MS, remember you can get your Sports physical now and turn it in to the Nurses Office here at Collins before the end of the school year.
